Insurance Protocol

Our staff will help you receive the maximum benefits available under your policy. All insurance cases require a pre-payment of the deductible, co-payment and any other non-covered benefits. In most cases, insurance does not cover the full cost of surgical care. It is designed to reduce your cost, but not eliminate it completely. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. 

If you have any questions regarding your specific coverage, you can contact your insurance company directly. If additional information is needed for your insurance company, we can provide diagnostic and procedure codes for your planned care.

  • For new patients, please fully complete and sign the Insurance Information page you have downloaded from our site (it is included in the Patient Registration Paperwork).
  • For existing patients, if you have not been to our office in a while you will need to update all your information and fill out new forms.  

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
